Enrollment Forms


The following are enrollment forms for Suttons Bay Public Schools:

 

Early Childhood Programs

  • If you are enrolling a child in our Lil Norsemen Program (Daycare-Preschool), please use this FORM.
  • If you are enrolling a child in our GSRP (Great Start Readiness) Preschool Program, please use this FORM.
  • If you are enrolling a child in our ECSE (Early Childhood Special Education), please use this FORM.
  • If you are enrolling your child in a Early Childhood Program or Kindergarten, please fill out this Health Appraisal FORM.

Elementary, High School and Early College

  • If you are enrolling your child in our Elementary, High School or Early College please use this set of FORMS
  • If you want to apply for the EARLY COLLEGE program, please fill out this FORM.
  • If you are enrolling more than one child in our Elementary, High School and/or Early College, please use the family  FORMS for your family information and complete this PACKET for each child you are enrolling.

Virtual School Programs

  • If you are enrolling your child in our Virtual K-12 School, please use this set of FORMS.
  • If you are enrolling more than one child in our Virtual K-12 School, please use the family  FORMS for your family information and complete this PACKET for each child you are enrolling.
  • If you are enrolling as a shared time student with our districts Virtual K-12 School or High School, please use this set of FORMS

Other enrollment forms

  • Are you interested in Suttons Bay Public Schools for your child/children for open enrollment? Please call us at 271-8600 for information. And here is the Application for the 2013-14 school year.
  • If your child is a Native American student with tribal affiliation, please fill out this FORM.
  • If your child has a current IEP for Special Education, please fill out this Temporary Placement FORM.
  • If your child has not completed all necessary immunizations, please complete this FORM.
